How to Apply for NDIS Plan Management with Coast & Country Primary Care
To get started, please make sure you:Have your NDIS number and a copy of your NDIS plan on hand so we can access the information needed to set you up.
Have Plan Management included in your NDIS plan under Capacity Building Support – Choice and Control or Improved Life Choices. If it’s not included, you can request it from your NDIA representative before registering.
Know if you’re on the new NDIS system (PACE). New participants will be notified during their planning meeting, and current participants will be informed during their plan reassessment. Don’t worry if you’re unsure — we can check this for you later.
If you are on PACE, please contact the NDIA on 1800 800 110 after completing your application to endorse Coast & Country Primary Care (NDIS Registration Number: 4050003930) as your Plan Manager.
We’re happy to assist with this step either over the phone or in person.
