Programs we deliver

Coast Nutrition is a free program that assists people to continue to live in their own home safely, comfortably, and as independently as possible by providing nutrition advice and support.

Coast Home & Garden Maintenance is a free service aimed at helping individuals maintain a home environment which supports wellness and reablement goals.

The program provides eligible residents access to free psychological therapy, also known as counselling or talk therapy. Our team is committed to providing professional, sensitive, evidence-based mental health care to strengthen your mental health and well-being.
Coast Nutrition

Coast Nutrition is a dietetics service that improves the nutrition and health of frail aged people, under the Commonwealth Home Support Program.
To help improve a person’s nutrition and health the dietitian can:Conduct a formal nutrition assessment
Provide nutrition information
Develop and deliver Medical Nutrition Therapy
Counsel and educate carers on dietary recommendations
Provide tips on food safety at home
Provide tips for meeting nutritional needs on a budget
Liaise with other health and community services

Katie is an Accredited Practising Dietitian for Coast & Country Primary Care and is responsible for the Coast Nutrition program. Having established the dietetic departments of two large local Allied Health organisations, she has extensive experience in providing community-based dietetic services.

Participants must:
Live on the Central Coast
Aged 65 years and over or an Indigenous person aged 50 years and over
Requires nutritional education and support
People with a disability can contact the Central Coast Community Pathways Team on 1300 106 096 to discuss access to this program.

Self-refer
Individuals can get access by calling My Aged Care Contact Centre on 1800 200 422.
Your GP can refer you by completing the online referral form at www.myagedcare.gov.au/referral
You may be referred by your hospital dietitian upon discharge. If your hospital dietician refers you and if you have not heard from the My Aged Care Assessment Team within 10-14 days, please call the My Aged Care Contact Centre on 1800 200 422 and ask for your referral to Coast Nutrition to be activated.
Once the referral has been assessed for eligibility, one of our accredited Dieticians will contact you. Please note referrals can take up to three weeks to be processed by the My Aged Care Assessment Team.

Coast Home & Garden Maintenance

Activities can include a range of maintenance or repair tasks such as:
Accessible, low maintenance garden redesign to support wellness and reablement goals
Minor plumbing, electrical and carpentry repairs where client safety is an issue.Repair of internal flooring and external access pathways to address slip and trip hazards.
Secure access issues for clients’ personal safety
Working-at-height related repairs or cleaning for client health and safety i.e. gutters, roofs, windows, ceilings, smoke alarmsYard maintenance – essential pruning, yard clearance or lawn mowing where there are issues for client safety and access.

In-Reach Program
Mental Health Support for Aged Care Residents
The program provides eligible residents access to free psychological therapy, also known as counselling or talk therapy. Our team is committed to providing professional, sensitive, evidence-based mental health care to strengthen your mental health and well-being.

The In-Reach Psychological therapy program provides residents experiencing mild to moderate mental health concerns with free one-on-one counselling sessions.

The In-Reach program is for people experiencing mild to moderate mental health concerns. If you are referred and wish to participate, one of our kind, caring, skilled mental health therapists will come to see you at the residential aged care facility.

The mental health therapist can see you up to 12 times over the next year free of charge. This is an optional support service; you can cease at any time.
The mental health therapist can also speak with your nominated caring family member or friend with your permission. The mental health therapist will update the facility manager, your GP, or the appropriate person at the facility.

A resident of an aged care facility or transitioning to a RACF.
The person has a diagnosed mental illness.
The illness is mild to moderate.

This program requires a GP referral or residential aged care facility.
