Improving the nutrition and health of aged people

Coast Nutrition is a free program that assists people to continue to live in their own home safely, comfortably, and as independently as possible by providing nutrition advice and support.

What does the dietitian do?

Coast Nutrition is a dietetics service that improves the nutrition and health of frail aged people, under the Commonwealth Home Support Program.

To help improve a person’s nutrition and health the dietitian can:

  • Conduct a formal nutrition assessment

  • Provide nutrition information

  • Develop and deliver Medical Nutrition Therapy

  • Counsel and educate carers on dietary recommendations

  • Provide tips on food safety at home

  • Provide tips for meeting nutritional needs on a budget

  • Liaise with other health and community services

Who is eligible?

Participants must:

  • Live on the Central Coast

  • Aged 65 years and over or an Indigenous person aged 50 years and over

  • Requires nutritional education and support

How to access

Self-refer

Individuals can get access by calling My Aged Care Contact Centre on 1800 200 422

  1. Your GP can refer you by completing the online referral form at www.myagedcare.gov.au/referral

  2. You may be referred by your hospital dietitian upon discharge. If your hospital dietician refers you and if you have not heard from the My Aged Care Assessment Team within 10-14 days, please call the My Aged Care Contact Centre on 1800 200 422 ask for your referral to Coast Nutrition to be activated.

Once the referral has been assessed for eligibility, one of our accredited Dieticians will contact you. Please note referrals can take up to three weeks to be processed by the My Aged Care Assessment Team.

You may also be able to use e-Referrals through your Medical Director or Best Practice management system as these are linked directly to My Aged Care.

Accessing Your Aged Care ID and Referral Code 

Where's My Aged Care ID? 

Find it after logging into your Online Account. Click the ‘Your Summary’ tile, and it's right below the navigation bar, under your name. Your Aged Care ID starts with AC followed by 8 numbers.

Locating Your Referral Code: 

Access your My Aged Care Online Account.
Simple visit myGov Home | myGov [my.gov.au].
Upon login, click the ‘Services’ tile to view eligible services and existing referral codes. 


Need to change providers or generate a new referral code?

You can reactivate codes or generate new ones in the portal.

Can't Access My Aged Care Online? 

Link your MyGov account to My Aged Care. Visit myGov Home or call My Aged Care at 1800 200 422 for assistance. 

Consumer Support from CCPC: 

  • Direct outreach post-hospital discharge. 

  • Assistance in accessing referral numbers provided by MYAC assessors. 

  • Guidance through the MYAC portal's "find a referral" section.

Katie Rae

Katie is an Accredited Practising Dietitian for Coast & Country Primary Care and is responsible for the Coast Nutrition program. Having established the dietetic departments of two large local Allied Health organisations, she has extensive experience in providing community-based dietetic services.
