Wise & Well

Build independence, capacity and improve overall wellbeing in seniors.

A free five-week group program for senior adults to connect, allowing for health assessments and information to be provided by health professionals to support healthy ageing at home.

Who is eligible?

Wise & Well is a free, nurse-led initiative available to Aboriginal and Torres Strait Islander people aged 50 or above and all other older Australians aged 60 who have at least one chronic disease diagnosis.

Funded by the Hunter, New England, Central Coast Primary Health Network, the purpose of the program is to provide early intervention programs that delay older Australians’ entry into Residential Aged Care Facilities (RACFs), reduce avoidable hospitalisations and improve equitable access to the services that older Australians require.

The program is aimed at supporting older Australians to live longer in the community by providing early intervention initiatives that promote healthy ageing, slow decline and support the ongoing management of chronic conditions.

Week 1: Assessments & Support Services

Information on support services available for senior adults on the Central Coast. Detailed assessments of health and wellbeing with the Registered Nurse. This may include onward referrals to external health professionals. We will discuss health literacy, elders’ rights and advocacy service education, and services available for carer support.

Week 2: Exercise & Falls Prevention

Led by an exercise Physiologist, assessment on falls risk, exercise tolerance, gait, and balance. Demonstration, participation, and education on exercises for everyone to continue with at home. Information on local specialists who can assist with mobility aids.

Week 3: Mental Health & Wellbeing

Keeping ourselves mentally well is a priority. Presented by a Mental Health Practitioner we will explore depression, grief & loss, loneliness, sleep, and mindfulness. Assessment and referrals to appropriate mental health services. A chance for guided meditation to manage stress and induce relaxation.

Week 4: Nutrition

Nutritional assessments and expert advice from a Dietician. Guidance on food safety and making nutritious meals on a budget and connection to ongoing services if required.

Week 5: Managing Chronic Conditions & Goal Setting

A look at how health impacts our daily living. What we can do to best improve, support and self-manage chronic health conditions as well as setting personal or health goals to motivate and keep you focussed. We will assist you to develop an action plan that will guide you toward your goals.
